* Introduction The lung transplantation patients with the diagnosis of COVID-19 had a high mortality rate that up to 41% in previous studies. However, the impact of the Omicron variant in lung transplantation patients was not well known.
*Objectives In this study, we focus on lung transplant patients who diagnosed with COVID-19 in Omicron surge, and we investigate the clinical presentations, outocmes and pulmonary function.
*Methods In this single-center retrospective study, we collected all the lung transplantation patients with the diagnosis of COVID-19 from Jan. 2022 to Dec. 2022. We collected the patient’s demographic data, laboratory data, and pulmonary function data from the electrical medical record. The pre-COVID-19 pulmonary function test was got from the closest routine test and the post-COVID-19 test would be arranged after being released from quarantine.
*Results There were 22 patients diagnosed with COVID-19 during this period. Four patients were admitted to the general ward for treatment and observation, and 2 patients needed oxygen support. All of the patients recovered from COVID-19. There was no significant decrease of forced vital capacity (FVC) (2.3±0.6 vs. 2.2±0.8 L, p = 0.363) and forced expiratory volume in the first second (FEV1) (2.0±0.6 vs. 1.9±0.8, p = 0.269) after COVID-19 infection. The daily prednisolone dose and the serum tacrolimus level were significant correlated to the duration of positive viral test and the change of FEV1. The patient did not receive fully vaccinated or booster dose had significant longer positive viral test duration (9.1±4.1 vs. 26.6±24.4 day, p = 0.05), and higher decline of FEV1 (23.6±137.3 vs. -331.8±537.8 L, p = 0.040).
*Conclusion(s) Even the lung transplant patients who diagnosed with COVID-19 in Omicron surge had low critical disease or mortality rate, we found the immunosuppressive medication and COVID-19 vaccination were correlated to the viral clearance and pulmonary function decline after COVID-19 infection. The ramdomised controlled study with larger case number is needed to confirmed them.

The future of intensive care specialists as new sub-specialties in Japan
Previously, Japan's medical specialist system was an accredited specialist system created independently by academic societies. However, starting in 2018, a new specialist system was introduced in which each academic society creates training programs for resident physicians from an academic perspective, which are then verified
and approved by the Japanese Medical Specialty Board (JMSB), which is connnissioned by the government. The JMSB-accredited medical specialist system is.a two-tier system(basic and subspecialty areas), and 19 basic areas and 27 subspecialty areas have been approved by 2022. Intensive care medicine is a subspecialty of the four basic areas (anesthesiology, emergency medicine, internal medicine, and pediatrics). In the spread of novel coronavirus infection (COVID-19) that began in 2020, the presence ofintensivists skilled in ventilatory management and ECMO management has played a major role in saving the lives of patients with COVID-19 severe respiratory failure and is now understood and recognized by many in the public as the hospital organizer of the best care for the patient. The mission of intensivists is not only to save the lives of critically ill patients but also to reintegrate them into society. To this end, intensivists must demonstrate leadership and play a role in providing the best possible medical care in collaboration and cooperation with physicians in various m(idical departments and healthcare professionals in various professions. The role of the intensivists is to provide the bestpossible medicalcare in cooperation and coIlaboration with pEysicians ofvarious medical depar1ments and healthcare professionals of various professions. Japan is an aging society, and many citizens will expect more than ever from intensivists who aim to improve quality oflife and reintegration into society. This symposium will provide an overview of the intensive care.medicine specialist system as a new subspecialty in Japan.

Effectiveness and Safety Profiles of Eravacycline in Real-World Setting

Implementing AI in clinical practice
Artificial Intelligence (AI) holds the promise of transforming clinical practice by providing tools for diagnosis, planning treatment strategies, and patient management. AI may be able to enhance diagnostic accuracy and offer precise predictions for patient outcomes. Additionally, it may be able to facilitate personalised treatment strategies derived from individual patient data.
However, AI's effectiveness hinges on vast data sets. It's crucial to maintain patient confidentiality and ensure data security. AI models, if not properly trained, can unintentionally perpetuate or amplify biases present in the training data. This can result in disparities in care or misdiagnoses for certain groups. Integrating AI tools with existing health information systems also presents challenges. Moreover, clinicians may not always be accountable for the decisions AI makes, especially in clinical decision-making.
While AI has found successful applications in certain areas, the key to its broader success lies in collaborative approaches. These should involve both AI specialists and clinicians in the development process. It's also essential to consider the potential economic impacts, including cost savings and changes in the workload of healthcare professionals.

Multiple organ failure is frequently observed in ICUs and strongly associated with poor outcomes. Severity of each organ injury and the number of injured organs are known to synergistically increase the mortality and multiple organ failure scores such as SOFA have been widely used so far. However, several modifications are necessary for SOFA score because of recent advances of medical devices such as ECMO. Furthermore, multiple organ failure may better be evaluated not only by summing up each organ injury but by analysing organ crosstalk among the injured organs; one organ injury will be compensated by another organ or one organ injury will cause another organ injury. We are trying to clarify the role of organ crosstalk both in basic and clinical studies.
