Despite the release of the new 2021 Surviving Sepsis Campaign guidelines, sepsis remains a complex disease with many unknown answers to the following questions. First, what impacts occur as a consequence of standardized sepsis care, taking into consideration timing, appropriateness and effectiveness? Second, what interventions enhance coordination and communication? Third, what devices or techniques can help clinicians manage complex decision-making in the setting of sepsis?With advance in machine learning and nanotechnology, we may be able to investigate new solutions to these complex problems.

This presentation offers a thorough review of the updated practice guidelines introduced in the past year, focusing on two intricate areas: Critical Care Medicine and Critical Care Nephrology.

Critical Care Medicine has seen groundbreaking progress in treatment protocols, particularly in patient ventilation, sepsis management, and post-ICU care. On the other hand, Critical Care Nephrology has made strides in areas like acute kidney injury management, dialysis techniques, and renal replacement therapies. The newly updated guidelines encapsulate these advancements, aiming for a better prognosis and improved quality of life for patients.

Our review dissects the salient features of these guideline updates, detailing the clinical implications and their potential impact on future care. This includes the rationales behind the shifts in recommendations, consolidating a bridge between past practices and present evolutions. We also delve into the methodologies employed in drafting these guidelines, emphasizing the rigor and scientific scrutiny involved.

Furthermore, by addressing future scopes, this presentation seeks to navigate the trajectory of these disciplines. Antimicrobial resistance (AMR) is a major and growing threat to global health. Infections by drug-resistant pathogens are associated with high levels of mortality and an increasing medical costs. Carbapenem-resistant A. baumannii, P. aeruginosa, and Enterobacterales are listed by WHO as critical priority for the needs of new and effective antibiotics. Several guidelines and guidance for the treatment of multidrug resistant organisms infections have been published worldwide, including Infectious Diseases Society of America (IDSA), European Society of Clinical Microbiology and Infectious Diseases (ESCMID), and Infectious Diseases Society of Taiwan (IDST).

TSCCM and TSECCM have collaborated with the Center of Outcome and Resource Evaluation (CORE) of the Australian and New Zealand Intensive Care Society (ANZICS), led by Professor David Pilcher. TSCCM and TSECCM build Taiwan CORE, which in the future can be used in evaluating the risk of death and peer comparison for improvement of the quality of critical care. Combined bundles of diagnosis and treatment are commonly used in clinical practice to treat complex diseases like sepsis. Therefore, the concept of combined artificial intelligent assistance models should also be brought into the process of intelligent critical care.
Integrating with combined early warning and dynamic prediction models, it will be able to continuously provide the latest information to remind various hazards and deterioration risks, assist the critical care medical care team to adjust diagnosis and treatment strategies in real time, and ensure critically ill patients to have the best outcomes.
